All accessories are included. The heatsink has been attached to the Raspberry Pi, and the fan and main unit have been assembled into the case. It will be delivered as assembled.
The 32GB MicroSD card has the 64-bit Raspberry Pi OS installed from Imager, not NOOBS.
You can use it immediately if you have a monitor, keyboard, and mouse.
